Study Closure or Withdrawal
Use the links below to quickly locate the appropriate final report form to close your research project. If your study involved an online survey created through the ORC's SurveyMonkey services, please remember to email ORC_Online_Survey@Mercer.edu and request the closure of any surveys associated with your protocol.
IRB Protocol Completion and Termination
It is the responsibility of the Principal Investigator to provide written notification of study completion within 30 days of the termination of all human subject research activities conducted under an approved protocol. Upon project completion, the Principal Investigator must submit a final report form the ORC to ensure the study is properly closed. When a study has been formally closed by the IRB, an official closure letter will be emailed to the Principal Investigator.
If the IRB does not receive a continuing review form before the protocol expiration date, the study will terminated. Once a study has expired or closed, all research activity involving human subject participation must cease.

IACUC Protocol Completion and Termination
Upon completion of all research activities, Principal Investigators conducting research involving laboratory animals must complete and submit the following IACUC protocol closure report.

If you are withdrawing yourself from a study, or if another approved research personnel associated with your study is withdrawing from the protocol, please download and complete the Personnel Amendment Form available on the Amendments and Modifications page.
